Cablecards keep coming unpaired

I got a new Bolt in December, and have had 4 cablecards since first install. Something is happening that is causing them to come unpaired. They work for about a month, and then I get the cablecard not authorized screen, and V58 errors on various channels.

At this point, Charter is pointing the finger at TiVo. TiVo says this must be a cablecard failure.

Has anyone else had a similar problem, and if so, what solved it? I'm pretty frustrated right now - I came home from vacation to find that...
