Loud screeching and subsequent loss of button sound FX

Last night the Tivo (Roamio +) started screeching loudly - it seemed to possibly be limited to the TOS link out but in any event after that the button sound effects stopped working regardless of going to the page and toggling back and forth from/to PCM. (We like to leave the sound FX on.)

It took a reboot to clear it out. It was particularly disturbing since it happened right in prime time while recording. I was a bit mystified at first since I had just hooked up and brand new A/V system...
