Missing channel number/name in season pass list

I have the following problem on my premier TiVo box (I have not noticed it on my Series 3 box). When I look at my season pass list, most of them show the program name and then in parentheses the channel number and channel name. However, there are a few which only have the program name (no channel number and channel name). I cannot figure out how I managed to create these entries without a channel number/name. I have NEVER used a wish list. All of my season passes have been created either by searching for a program name or by finding a program in the live TV guide and selecting it. Can someone please tell me how I managed to create these entries so that I can avoid doing so it the future? Thanks.
