Different guide data in app vs on TiVo - recordings being cancelled

Three times since the guide update I'm set a recording in the TiVo app only to find if didn't record on the TiVo. When I looked in the history it either lists another program and "cancelled" in the listing or it has rescheduled some random program in its place.

Has anyone else seen this occur? It's ruining all my recordings
